Malignant Hearts were added in Season of the Malignant, and players can capture these to obtain Caged Hearts, which provide different bonuses.
Each Caged Heart focuses on either Defense, Offense, Utility, or Ultimate bonuses that players can utilize to create new and deadly builds for taking on the monsters in Sanctuary.
Some Caged Hearts can be used by all classes while some are restricted to a certain class, and we have listed everything down here for your convenience.
How To Get Caged Hearts?
There are several ways for players to obtain Caged Hearts and these will often result in you getting random ones regardless of what you do.
The following are the ways for you to get Caged Hearts:
- Capturing Malignant Hearts from Defeated Malignant Elites
- Using a Malignant Invoker on a Malignant Outgrowth
- Crafting the Caged Heart at Cormond’s Workbench

Malignant Hearts for All Classes
Damage over time effects is increased by a certain percentage for each Crowd Control effect a target suffers. Staggers are Unstoppable monsters. bosses instead take increased damage from your damage over time effects.
Resources draining effects become less effective while you gain increased Resource Generation.
After losing more than 20% of your Life in a single hit, you become Immune for a short duration. (110-second Cooldown)
After a Crowd Control effect is removed, you deal Fire damage to surrounding enemies.
Suppresses a percentage of incoming damage. Using a Defensive, Subterfuge, or Macabre skill causes all suppressed damage to be amplified and explode to deal Fire damage to nearby enemies.
Critical Strike Damage is increased while Non-Critical Strike damage is reduced.
Critical Strikes and any additional damage afterward within a short duration are absorbed by your target. The damage that is absorbed will erupt and damage the surrounding enemies. Any damage that is stored will increase by a certain percentage per second.
Whenever you spend a certain amount of your Primary Resource, your next attack is to stun enemies.
|The Dark Dance||Vicious|
Every 5 seconds that go by that your Life is above 60%, your Core Skills will use Life instead of your Primary Resource. Dealing damage with these skills will deal an extra amount of damage.
Increases your Barrier Generation and provides you with Life regeneration per second while a Barrier is active.
The Malignant Pact
|Wrathful||Cycles through different Malignant bonuses every with every 20 kills, providing you with the following:|
Critical Strikes charge enemies for a short duration, causing all affected enemies to suffer from lightning that arcs between them, dealing Lightning Damage.
Barbarian Malignant Hearts
|Spending a certain amount of Fury within a short period will cause your next Non-Basic Skill to have an increased Critical Strike Chance.|
|Provides the chance to ignore incoming damage and heal you instead.|
|Provides your skills with a chance to Knock Down all enemies for a short duration when their attack speed is higher than a certain percentage.|
|Having a certain amount of reduced Life will cause you to receive increased healing from all sources.|
Druid Malignant Hearts
|Your kills have a chance to summon a Wolf Companion to your side for a certain duration. Additionally, you gain +3 to Wolves.|
The Agitated Winds
|When there is a certain number of Close enemies, you will automatically cast Cyclone Armor.|
|A certain number of Distant enemies will be pulled toward you when you have your Ultimate Skill active.|
The Unconstrained Beast
|Being hit with a Stun, Freeze, or Knock Down effect provides a chance to automatically activate Grizzly Rage for a short duration.|
Rogue Malignant Hearts
|Lucky Hits provide you with a chance to launch Stun Grenades that deal Physical Damage and Stun enemies for a short duration.|
|Using a Subterfuge Skill will leave behind an unstable Shadow Decoy Trap that Taunts enemies. This trap explodes after a short duration, dealing Shadow Damage in an area.|
|Lucky Hits provides you with a chance for your Cutthroat Skills to Slow by enemies for a short duration and your Marksman Skills to Knock Back targets.|
The Vile Apothecary
|Your attacks have a chance to apply all Imbuement effects at a certain percentage of their normal potency.|
Necromancer Malignant Hearts
|When walking by a Corpse, a Corpse Skill will be activated every second, dealing a percentage of reduced damage.|
The Decrepit Aura
|You gain an Aura that casts Decrepify when there is a certain number of enemies near you.|
|Lucky Hit has a chance to inflict Fear for a short duration and additionally, those who are Feared become Chilled for a short duration.|
The Great Feast
|Drains your Essences per second but increases your Minion Damage. Works similarly on you if you have no Minions but more Essence is drained.|
Sorcerer Malignant Hearts
|Each element you deal damage with provides you with increased damage for a short period.|
|Taking Elemental Damage causes you to gain Resistance to that element for a short duration.|
|Being affected by Crowd Control effects provides you with a chance to spread the effect to enemies around you for a short duration.|
|Core Skills that send out projectiles consume all of your Mana, causing an additional projectile to be launched based on how much was consumed.|
There are plenty of Caged Hearts to go around and eventually, you will be able to collect all of them for each class if you play enough with them.
Some classes can make great use of their specific Malignant Hearts while shared ones are a perfect addition at times.
Not every build requires all of the Malignant Hearts though and you can only use 3 at a time since they only fit into Jewelry, making it important to choose wisely on what you use.
