It has been reported that this will not happen if you defeat the Echo of Varshan on World Tier 3 or higher, which makes it necessary to reduce the World Tier first.
Once the Echo of Varshan has been slain in a Malignant Tunnel in World Tier 1 or 2, he will drop the Foul Invoker Of Varshan.
