A new specimen was obtained after you previously obtained Vex Code Mutations and brought them back just before activating Failsafe’s Upgrade Protocol.
The specimen is related to one of the Vex Hydras that you have been tracking and for some reason, the Fallen are trying to capture it.
You will need to fight your way through various enemies to find the Vex Hydra and obtain a specimen from it before the Fallen succeeds.
How To Get Specimen ID: NES004 Quest?
The Specimen ID: NES004 Quest becomes available to players during quest step 5 of A Rising Chorus: Act 2.
You may receive this quest after speaking to Failsafe upon the completion of quest step 4 where you previously activated her upgrade protocol.
Specimen ID: NES004 Quest Steps
Step 1: Open Echo Chests at the end of Episode: Echoes activities in the H.E.L.M. To decrypt the algorithm
This quest step requires that you participate in Echoes activities and open the chests that appear at the end. (Breach Executable is a good choice)
Each Echoes chest that you open will reward you with 44% progress.

The description for this quest step doesn’t help much but all that you need to do is focus Echo Gear, which should help replicate the signature.
You will need to focus Echo Engrams into any type of gear 5 times to complete this quest step.

Step 3: Secure Extracted Date for Failsafe in the Breach Executable activity and fight your way through Echoes Battlegrounds
To complete this quest step you will need to Extract Data during the Breach Executable activity and defeat Powerful Combatants in the Echoes Battlegrounds activity.
This can be completed by running a single Echoes Battlegrounds activity, which you can start from this quest’s menu or the H.E.L.M.

Step 4: Speak with Failsafe in the H.E.L.M. to review her final specimen analysis report
Head back to the H.E.L.M. and speak with Failsafe, who will reward you with the Mirror-Encrypted Hydra Tail.
This unlocks the Armor Reputation Booster, which increases the reputation earned through the Seasonal armor perk by 100%

Specimen ID: NES004 Quest Rewards
Completing Specimen ID: NES004 will provide you with various loot after you complete the activities that you do during the quest.
Upon completion of this quest, you will be able to add the Mirror-Encrypted Hydra Tail to your collection aboard the H.E.L.M.
This also unlocks the Armor Reputation Booster, which increases the amount of Reputation you get from the Seasonal Armor Perk by 100%.
Summary
Failsafe was able to locate the location of the Vex Hydra but the Fallen had already begun to make their attempt to capture it.
