After you and Locran arrived at the Ruins of Rakhat Keep, you discovered the area crawling with demons and the heretics gone but evidence was left behind.
The remains of Sankekur lie within the Black Tomb, which is located somewhere within the Foul Undercrypts, which is accessible from the Ruins of Rakhat Keep.
The two of you must now search for the Black Tomb to find the remains of Sankekur to banish his Hatred to hell to stop the heretic’s plan.

Inside the Black Tomb Quest Walkthrough 
After reaching the Ruins of Rakhat Keep, you and Locran found evidence of the heretics’ plans and now need to find the remains of Sankekur to put a stop to them.
The entrance to the Foul Underkeep is nearby and you will need to interact with the Horadric Seal so that Locran can open the entrance.
Once the entrance has been opened, the two of you will need to explore The Black Tomb and find a way to reach the remains of Sankekur.
After fighting your way to The Black Tomb, speak with Locran and he will begin a ritual to open the entrance within the tomb.
Locran will give you his talisman, which should protect you from the Lord’s Hatred, but he claims that he does not know how long this will last.
You are now to enter the tomb, which is labeled as “Descend into the Unknown”, and this will set you on the path to finding the remains of Sankekur.
Descending into the unknown will take you to the Sanctum of Stillness, where you and Locran will search for parts of Sankekur.
This will be done by placing Locran’s Talisman and defending him as he performs a ritual to make nearby urns accessible to get the Ear and later the Tongue of Sankekur.
The two of you will need to continue until you reach Sankekur’s remains and once again, Locran’s Talisman must be placed and Locran will perform a ritual.
Upon reaching the remains, you will need to collect one more piece of Sankekur which is the eye but investigating the urn reveals that it is missing.
Locran becomes frustrated but after a moment of silence, he removes his eye as a sacrifice and gives it to you.
Your next step is to place Locran’s Talisman and reunite the pieces (including Locran’s Eye) with the remains of Sankekur.
Locran will perform a ritual, and you will need to defend him from The Corpse of Sankekur and the enemies that appear in the area.
An enemy called Maffer the Cruel will appear and you will need to defeat this demon, which is simply a taste of the enemies you will face from the Infernal Hordes.
Locran will be weakened and reveals that he saw the doom of Sanctuary and tells you to meet him outside so you both may speak.
You will need to exit the dungeon that you are in and meet Locran outside, who can be found weakened on the ground near a torch.
Speak with Locran and he will mention that there is still one way to prevent hell from spilling into Sanctuary, which involves you fighting them.
After your conversation, the two of you will have plans for a final stand that will have been decided and the quest will come to an end.
Inside the Black Tomb Quest Rewards
Once you have exited the Foul Undercrypt and have spoken with Locran, the quest will end, and you will receive the following rewards:
- Item Cache
- Sacred Legendary Equipment
- XP
- Gold
Verdict
The attempt made by you and Locran to banish the Hatred failed and it seems that the heretics pulled through with their plans.
After barely making it out alive, you and Locran have one last option to keep hell from spilling out and the Sanctuary being doomed.
This will be to fight the demons on their turf, which means you will need to go to the Hell Gate and fight their forces head-on.
