The Blood Maiden acts as a type of field boss that players may encounter in the Helltide by starting the Accursed Altar activity.
This demon is also encountered in the Season of Loot Reborn storyline but is mainly fought repeatedly when players explore the Helltide.

How To Summon Blood Maiden?
Summoning a Blood Maiden requires that you head into the Helltide and find an Accursed Altar, which can be spotted on your map by looking for the icon of a head with horns.
Once you have found the altar, there will be 3 parts of the altar that you need to interact with but this will require you to have Baneful Hearts.
This requires that each of the altars be provided with a Baneful Heart and once done, the summoning ritual will begin.
How To Get Baneful Hearts?
You may get Baneful Hearts by fighting demons to collect Aberrant Cinders, which can be used to open Tortured Gift of Mysteries chests.
Another way is to continue fighting as many enemies as you can to increase your threat level, which is represented by a meter next to your map.
Once your threat level has been filled up, you will be able to fight Hellborne enemies who may drop Baneful Hearts.
How To Defeat Blood Maiden?
The Blood Maiden is relatively easy to defeat if you are prepared and fight back against the other demons that come along with it.
This demon will move around often and attack you just like any other, but it also flies out of range to summon multiple Hellborne to attack you. (Becomes invulnerable for a short duration)
Clearing out the other enemies is a good way to allow you to focus on the Blood Maiden, especially since taking on too much damage can lead to your death.
It is an idea to focus your attacks on the Blood Maiden and clear out any other enemies whenever you don’t have a chance to damage it.
Blood Maiden Rewards
Blood Maidens will drop various loot when they are defeated and these will always be randomized from items within their drop table, which may include the following:
- Aberrant Cinders
- Equipment
- Temper Manuals
- Crafting Materials
- Aberrant Cinders
These rewards may come in different rarities depending on your level as well as the World Tier that you are currently playing in.

Blood Maidens on their own are easy to defeat but that does not mean it will be a walk in the park as they do have some tricks up their sleeves.
One of the actions they do that stalls their defeat is that they fly around and summon Hellborne enemies, which may sometimes deal significant damage if ignored.
It is ideal to use up your Baneful Hearts to farm Blood Maidens to get a lot of loot during the season as they have a lot to offer, especially better equipment.
