Upon reaching Torment I, there will be several bosses that you will face off against and when it comes to the Greater Tier of Lair Bosses, certain Lair Boss Keys are needed.
One of the Lair Boss Keys that you will need is the Pincushioned Doll, which is required to obtain better rewards from the Hanged Man’s Hall where you fight Andariel.
Back then, Pincushioned Dolls were one of the Boss Summoning Items that you need but this changed later in Season 8 where other Boss Summoning Items were turned into Lair Boss Keys.
How To Get Pincushioned Doll?
Pincushioned Dolls do not drop as often as Initiate Lair Boss Keys and require you to go through more challenging activities throughout the season.
Some of the best ways are to defeat the bosses from the Glacial Fissure (Beast in the Ice) or Ancient’s Seat (Lord Zir) and open their Hoards as this can reward you with Pincushioned Dolls.
Other methods for obtaining Pincushioned Dolls involve defeating World Bosses that spawn in the Sanctuary or using a Tribute of Titans during the Kurast Undercity activity.
Pincushioned Doll Uses
Pincushioned Dolls are used to open Andariel’s Hoard Chest after you have defeated her in the Hanged Man’s Hall dungeon. (Boss Lair)
Opening Andariel’s Hoard Chest will provide you with valuable loot, which may include items from the following loot table:

Verdict
Farming Pincushioned Dolls may be difficult for players who have not reached Level 80 to 85 as The Beast In The Ice can be quite the challenge.
It is best to make sure you have equipment that provides you with resistance to Cold Damage or use an Elixir of Cold Resistance.
Since more than one Pincushioned Doll is needed to face the Echo of Andariel, it would be wise to prepare enough Distilled Fear for multiple runs.
