Taking on Varshan within the Malignant Burrow requires that you first gather Malignant Heart, a Lair Boss Key used to open his Hoard.
Back then, you needed to gather several items to summon Varshan but this can now be done without the Boss Summoning Materials.
To obtain a ton of loot from Varshan however, you will need to have enough Malignant Hearts to unlock Varshan’s Hoard Chest.
How To Get Malignant Heart?
Before getting your hands on any Malignant Hearts, the first thing you will need to do is switch over to Torment I, which is the start of the endgame phase of any season.
Once you are on Torment I or higher, you may obtain Malignant Hearts by opening Tree of Whisper caches or running the Kurast Undercity activity using a Tribute of Titans.
Other ways to obtain Malignant Hearts are to join other players and defeat World Bosses or to opt for Mercenary Bartering which happens in The Den.
Malignant Heart Uses
Upon defeating Varshan within the Malignant Burrow, he will drop minimal loot and shortly after he falls, a chest named Vasrshan’s Hoard will appear.
Opening Varshan’s Hoard requires 12 Malignant Hearts (Varshan Lair Boss Keys) to open the chest, will drop loot with a chance to have some items from the following drop table:

Verdict
Varshan can be quite a tough enemy to defeat due to the minions that he spawns to sustain himself but the rewards earned are worth the trouble and you don’t lose anything if you fail.
You will need to have at least 12 Malignant Hearts or else no rewards can be obtained from Varshan’s Hoard, which makes it necessary to farm them beforehand.
Unlike in the past, getting rewards from Varshan was a tedious task but now you can easily farm his Hoard thanks to the Lair Boss Rework that was released in Season 8.
