Distilled Fear is a Lair Boss Key that was previously a Boss Summoning Item until the Lair Boss Rework that took place in Season 8. (Belial’s Return)
This is one of the Lair Boss Keys that you may obtain, which is used later when you have defeated The Beast in Ice to access its Hoard Chest.
Distilled Fear is one of the more common Lair Boss Keys that you will obtain once you start making your way through the endgame part of seasons before you take bosses from a higher tier.
How To Get Distilled Fear?
Distilled Fear will often drop once you are on Torment I and can be rewarded for completing Seasonal Activities along with regular endgame activities such as Nightmare Dungeon.

Distilled Fear Uses
Distilled Fear is used to open The Beast in Ice’s Hoard chest, which appears after the boss has been defeated in the Broken Mirror within the Glacial Fissure dungeon.
You will need to use 12 Distilled Fear to open The Beast in Ice’s Hoard chest, which may include drops for the following loot table:

Once The Beast in Ice has been defeated, you will be able to access its Hoard Chest that will appear nearby and this requires 12 Distilled Fear each time you attempt to open it.
Starting from Torment I, Distilled Fear will be a possible drop from Nightmare Dungeons, and doing these will also allow you to farm other items such as equipment and even Obducite.
Saving up on Distilled Fear is ideal since opening The Beast in Ice’s Hoard Chest is one of the ways to farm other Lair Boss Keys, including those from a higher tier.
