Reaching this point requires completing the Season Rank V Capstone, which takes you through the Worldstone Keep Waking Nightmare on Torment IV before allowing you to claim the rank rewards.

After entering the portal, you will arrive in Whimsyshire with the rather vague objective “Come Play With Us,” leaving you to explore the area and find your way through it.
Make your way through Whimsyshire while defeating the enemies you encounter, including Cuddle Bears, unicorns, walking flowers, and other creatures scattered throughout the level.
There is no complicated objective to complete while exploring, so continue clearing enemies and following the available paths until you eventually reach the Den of Whimsy.
Entering the Den of Whimsy begins the final part of the level, where your objective changes and you must defeat Neighrelle to complete Whimsyshire.
Whimsyshire Interactive Objects
Several objects found throughout Whimsyshire can be interacted with or destroyed for additional loot, so exploring the surrounding paths can be worthwhile during your visit.
Happy Clouds can be interacted with to release rewards, while Candy Jars, Soup Chests, Weapon Gifts, and Armor Gifts can also be encountered throughout the secret level.

How To Defeat Neighrelle?

Keeping some distance gives you more room to react, while dodging around the boss allows you to avoid its larger attacks before moving back in whenever you have an opening.

Whimsyshire Rewards
Whimsyshire provides plenty of regular loot while exploring, including Gold, equipment, crafting materials, Charms, Primordial Dust, and other resources from enemies and interactive objects.
Defeating Neighrelle completes the secret level and also unlocks the Fancy Pants player title, giving you a unique reward for finishing the entire run at least once.
There is currently no confirmed guaranteed Mythic Unique or similar exclusive equipment drop from Neighrelle, so the main unique reward appears to be the title rather than a specific piece of gear.
Can You Return To Whimsyshire?
The Liquid Rainbow is consumed when used, meaning the guaranteed Season Rank V copy does not provide unlimited access to Whimsyshire.
