Lair Bosses remain one of the most reliable ways to target specific Unique Items while progressing through Diablo 4’s endgame.

Belial (Palace of the Deceiver)
Belial’s Hoard functions differently from other Exalted Lair Boss rewards, allowing you to choose a specific boss loot table when claiming your rewards.
This makes it easier to target farm certain Unique Items, as you can select the boss associated with the gear that you are looking for.

How To Obtain Loot From Lair Bosses?
Defeating a Lair Boss causes a Hoard to appear, which can be opened to claim the main rewards from that encounter.
Initiate, Greater, and Superior Lair Bosses require their corresponding Lair Keys to open their Hoards and obtain the available loot.
Bosses can still be defeated without a key, but their Hoards cannot be opened until the appropriate Lair Key is available.
Target Farming Unique Items
Each Lair Boss has a dedicated selection of Unique Items, allowing players to repeatedly farm a particular encounter when searching for specific equipment.
The first Unique obtained from a Lair Boss Hoard is guaranteed to come from that boss’s dedicated loot table, giving players a reliable way to narrow their search.
Additional Unique Items can come from the broader Unique pool, meaning equipment outside the dedicated boss table may still appear among the remaining rewards.
Belial provides additional flexibility by allowing players to select another Lair Boss’s loot table when opening his Hoard instead of having a conventional dedicated table.
