With the entrance to the Durance of Hate within meters away from you, the time has come to enter it and search for Neyrelle.
This leads you to fight your way to reach her and while she is present, Mephisto seems to also influence what is happening.
You will need to rush to Neyrelle in hopes of helping her before the worst is to come but not everything goes the way you expect it to.
How To Get The Heart of All Rot Quest?
The Heart of All Rot begins once you have completed the quest called A Flickering Light, where you previously spread light with Eru’s ritual to gain access to the Durance of Hate.
You and Eru are now bound to enter the Durance of Hate where the two of you will continue your search for Neyrelle.

- Search the Durance of Hate with Eru for signs of Neyrelle
- Open the Sealed Door
- Survive the Hollow assault
- Speak with Eru outside Durance of Hate
The Heart of All Rot Walkthrough
You and Eru continue your journey by entering the Durance of Hate, where the two of you will fight your way through enemies until you reach a Sealed Door.
Opening the Sealed Door will begin a cutscene where you find Neyrelle chanting as she wields Mephisto’s Soulstone and it appears that she has been tainted.
Mephisto makes an appearance and by the looks of it, he has grown stronger given that his wolf vessel has become larger.
After the cutscene, you and Eru will be attacked by Hollows and will need to survive the encounter by defeating all of them, including an elite called Vesicator.
After defeating Vesicator, a mighty lion-like spirit appears (Spirit of Akarat) who instructs you to seek out Ah Bulan, who Akarat says may help her.
The quest ends shortly after the Spirit of Akarat makes its appearance, which also results in a portal being opened up.
The Heart of All Rot Rewards
Once you have escaped the Durance of Hate and spoken with Eru, the quest will be complete and you will be rewarded with XP and Gold.
A new quest called To Kurast will begin, which puts you on a path to Kurast, a familiar name to those who remember the past Diablo Titles.
Verdict
You were able to get a glimpse of Neyrelle but it seems that Mephisto’s Hold on her is greater than you may have thought, meaning he is getting stronger.
While there may have been an ambush, the Spirit of Akarat intervened for a short moment and you were able to escape.
You are now to make your way to Kurast and find a way to seek help from Ah Bulan, who Eru believes may be able to help Neyrelle.
