Fate Laughs at Us All is the next quest after you have completed the Faith and Failings quest, where you previously escaped from the crowd during the sermon.
Akarat, who is still possessed by Mephisto, continues to spread their influence and has even turned the public against you, making it nearly impossible to make your move.

After escaping from the crowd, follow the waypoint to reach the observatory, and once you arrive, interact with the door to open it.
You will find that Queen Adreona is arguing with Oracle Cyra, who refuses to aid her since Akarat’s influence has already gotten to the queen.
Speak to Tharra, who calls out to you, and this will begin a cutscene where the oracle attempts to convince the queen, but this fails.
Tharra will attempt to speak out to the crowd, but the results remain the same, and after this happens, you will need to speak to the queen.
Speaking to Queen Adreona begins another cutscene, and she recognizes you, which results in her blaming you for what has happened to Skovos.
The majority of the people in the area leave, and you are left to speak with Oracle Cyra, which eventually ends the quest afterward.

Speaking to Oracle Cyra after Queen Adreona has left will complete the quest, which starts a new one called On The Hunt.
