With the second piece of Lilith’s Blade now within your possession, only one remains unfound, and you will now make your way to Atanos to look for it.
Tharra will be waiting for you in Atanos, and you must now make your way there, but it looks like Akarat, who is still possessed by Mephisto, has arrived ahead of you.
You must meet up with Tharra and see what can be done regarding the last piece of the blade while also avoiding whatever problems Mephisto may cause at the same time.
How To Get Faith and Failings Quest?
Faith and Failings becomes available once you have completed the Glint of Light quest, which took place back at Athulua Garrison.
Now that the search for the third part of the blade has begun, it is up to you and Tharra to continue the journey while the others remain at the garrison.
Faith and Failings Walkthrough
Once the quest begins, make your way to the marked area where you will find Tharra and speak to her before the two of you proceed further.
Fight your way to the next marked location, and you will find pirates pillaging dead bodies, and you must clear them before moving to the next area.
The next area is where you will find an Amazon Sentry, who speaks of Akarat nearby, who is about to begin his sermon.
Instead of joining Tharra to go speak with Queen Adeana and the Oracle, you decide to stay behind and hear what Akarat has to say to the people.
Once Tharra leaves, continue by making your way to the sermon, and as you get closer to Akarat, more of the people will move out of the way.
Continue making your way to Akarat and speak with him, which results in a cutscene where he turns the people against you.
Leave the area and make your way to the Celestial Observatory to catch up with Tharra, and once you arrive at the marked location, the quest will be complete.
Faith and Failings Quest Rewards
After escaping the crow and making your way to the Celestial Observatory, the Faith and Failings quest will be complete, resulting in a new quest called Fate Laughs at Us All to begin.
In addition to the next quest being unlocked, you also receive:

Your attempt to secretly observe Akarat’s sermon quickly turned against you as Mephisto manipulated the crowd and turned the people hostile in an instant.
Although the situation became dangerous, you managed to escape before things spiraled any further, and it appeared Mephisto had no intention of attacking just yet.
After making your way to the Celestial Observatory, the focus now shifts toward the search for the final piece of the blade and whatever Tharra has discovered ahead.
