After splitting up with Lorath in the Temis Promenade, you encounter a Hand of Akarat and decide to follow him until you reach the Temis Sewers.
It is uncertain what lies within these sewers and why the Hand of Akarat would have made his way there, leaving you to find out.
You will need to enter the sewers and pursue them to find out what they are up to before meeting up with Lorath, wherever he may be.
How To Get the Subterranean Secrets Quest?
Subterranean Secrets becomes available once you have completed the Between Heaven and Hell quest, which leads you to the entrance to the Temis Sewers.
Subterranean Secrets Quest Walkthrough
You will be starting by entering the Temis Sewers and will need to explore it as you push forward to find the Hand of Akarat, who recently entered.
Once you are in the sewers, you will come face to face with various rodent-like monsters and will need to fight your way past them.
Fight your way to the end, and you will stumble upon the area where the Hand of Akarat has gone, which seems to be a place where a ritual is happening.
Speak with the Hand of Akarat, and shortly after, he, along with the other members in the room, will sacrifice themselves, spawning a Galvanized Mauler.
The Galvanized Mauler will be a tough enemy to defeat, and it is advisable to avoid its attacks whenever you can as you continue to fight it.
After defeating this enemy, continue making your way to the exit, which will lead you to the Plaza of Queens, specifically the Plaza Overlook.
Once you reach the area marked by a waypoint, the Subterranean Secrets quest will shortly come to an end.
Subterranean Secrets Quest Rewards
Upon reaching the Plaza Overlook, the quest ends, and a new one called The Queen and the Saint will begin.
In addition to the next quest being unlocked, you also receive:

Reaching the Plaza Overlook brings things back above ground, but it’s obvious now that what’s happening beneath Temis is only part of something much bigger.
