Your trip to Mount Karcheus brought you on a journey within yourself as you peered into the Sightless Eye, giving you a chance to reach what remains of Lilith within your blood.
You are brought into the Mind’s Eye, and it is here that a part of Lilith dwells, which requires you to find her while you are still in control.
While within the Mind’s Eye, you will need to search for Lilith and see if you can convince her to help you put a stop to Mephisto.
How To Get She Sleeps Within You Quest?
She Sleeps Within You becomes available after you have completed the quest called In The Shadow of Another, which had you prepare the Sightless Eye and gaze into it.
This quest begins immediately after a cutscene that plays when you interact with the Sightless Eye, instantly completing In The Shadow of Another.
She Sleeps Within You Walkthrough
Starting in The Mind’s Eye, you will need to explore the area and will eventually come across blood petals, which are the remnants that Lilith often leaves behind.
Continue following the trail of blood petals until each destination is reached, and this will lead you to a few objects that you need to interact with.
You will eventually be led to a doorway called The Threshold, which you will need to interact with and afterward, enter.
Push forward into the next area, fighting your way past the harsh wind inside until you reach Lilith inside a chamber at the end.
Speak with Lilith, and she will refuse to help at first, but later reconsiders and begins unraveling Fragments of a Memory in the chamber, which you need to interact with.
After interacting with all 3 of the Fragments of Memory, speak with Lilith, and after a brief conversation, she will warn you about being hunted, requiring that you leave.
She Sleeps Within You Quest Rewards
After speaking to Lilith, She Sleeps Within You will be complete, and you will gain a new quest called Tipped Scales.
In addition to the next quest being unlocked, you also receive:

She Sleeps Within You deepens the narrative in Diablo 4 by bringing you face-to-face with Lilith in a tense and uncertain encounter within the Mind’s Eye.
The conversation reveals more about Mephisto’s plans, especially his interest in the Pool of Creation and what he may intend to do.
Though initially dismissive, Lilith reconsiders after hearing about Skovos, but the exchange is cut short as you are hunted and forced to escape.
