Together with Eru and Neyrelle, you were able to find Akarat’s final resting place, and just as things seemed to be going well, the worst happened.
Eru has betrayed you and after surviving the Harbinger of Hatred that was released upon you, searching for him is your next goal.
Accompanied by Neyrelle, you have taken a portal to Reflection’s Edge and will need to hunt down Eru to find answers and make him pay for what he has done.
How To Get Catharsis Quest?
Catharsis is one of the quests of Chapter 4 that begins once you have completed All Good Things, which involves you finally reaching Akarat’s Burial Chambers.
Neyrelle was successfully cured of Mephisto’s corruption but in the end, Eru made off with Mephisto’s Soulstone and Akarat’s body.

Catharsis Walkthrough
Once you have reached Reflection’s Edge, you will need to make your way to the marked area to continue your search for Eru.
Upon reaching the marked area, Neyrelle notices blood on the ground and you will need to speak to her to continue, which starts a cutscene.
During the cutscene, it is revealed that Eru has made a deal with Mephisto, which is to spare everything that he loves from his rule.
Regardless of Neyrelle asking about the Soulstone and Akarat’s body, Eru refuses to cooperate and instead demands that you kill him but Neyrelle knocks him out instead.
Catharsis Rewards
After the cutscene where Eru is knocked out as a way of letting him go, the Catharsis Quest will be complete and you will be rewarded with XP and Gold.
Additionally, one of the rewards you get will be a Legendary piece of gear and in our case, this was a Legendary Helmet. (Bruiser’s Hatred’s Gaze)
Upon completion of Catharsis, the story continues with a quest called To Walk a Different Path being active.
Verdict
After exiting the portal, tracking Eru down was not that difficult as he had left a trail of blood and was not able to make it that far.
Eru had felt ashamed for what he did and attempted to attack you to force you or Neyrelle to kill him but Nyrelle spared him so he could live with his guilt.
All seems lost but there is more that can be done and you will need to meet with Neyrelle at the Horadric Vault to figure out what to do next.
